After Inarius banished Lilith to the Abyss for slaughtering the renegades that called for the killing of all Nephalem to protect themselves and Sanctuary, he altered the Worldstone so that over time their power would diminish. The Nephalem themselves felt their power draining by the day essentially. Some protested about this decision, but were killed by Inarius. Over the generations, the Nephalems’ power continued to dwindle until it faded completely.
Something to note is that for a time there were still some Nephalem remaining while normal humans were around, until they too died off or lost the remaining vestiges of their power. So it wasn’t just a blanket “everyone depowered all at once” thing.
After the destruction of the Worldstone, 20 years went by before D3 even starts. And the player character starts off fairly wimpy and by the end, mantles their birthright. Another thing to note is “The Temple of the Firstborn” was used to by a blood cult lure in OTHER Nephalem and drain their blood for power by the demon Vidian. Soooo by the end of D3 there are actually other Nephalem out there slowly regaining their powers.
So honestly, Diablo 3 is just a mirroring of the actions of Inarius, slowly unraveling.
During the main events of Diablo 3, you ARE the only Nephalem, AT THE MOMENT. But others are slowly beginning to stir it seems.
Another thing though - you don’t want the Nephalem powers to come back instantly, it would end badly. When Lilith found a way to escape the Abyss after being sent there by Inarius, she “unlocked” the potential of Uldyssian Ul-Diomede letting his powers begin to return. The process starts slowly at first, but after a certain point, the accumulation of his total power began to accelerate to such a degree that it threatened to literally destroy Sanctuary itself. We’re talking reality warping, resurrection, infinity gauntlet with all the stones kinda shit. So he had to draw in all the energies into himself and send himself to the abyss to release the energies which negated his entire being. During the process the Worldstone was altered to suppress the powers of the Nephalem again. This sacrifice is what causes Tyrael to see the good in humanity and be the tie breaking vote to not destroy Sanctuary.
